K-12 Registration
VCSD is committed to providing registration information in ways that are accessible to all. The registration packet is offered through three options: as a series of fillable forms, a downloadable packet, or through pick-up at the registrar’s office.
Please note: children must be five years of age on or before December 1, 2026, in order to register for kindergarten in the 2026-2027 school year. In addition to filling out the registration packet, parents and guardians of students entering kindergarten are asked to fill out the Kindergarten Screening Questionnaire .
Option 1: Register using fillable forms
Parent and Guardian Forms
Health Exam Form (to be completed by your student’s medical provider)
Dental Health Certificate (to be completed by your student’s dental provider)
Affidavit of Residency (to be completed by parent/guardian and notarized)
For this option, forms must be printed out after they are filled in.
Option 2: Download and print registration packet

For this option, forms are printed first and then filled in.
Option 3: Visit the registrar’s office
Parents and guardians are also welcome to pick up printed copies of all registration material at the Registrar’s Office between the hours of 8 a.m. and 4 p.m. Monday through Friday.
All three options contain the following components:
- Letter from principal indicating which documents you will need to enroll your child
- Special Education Services
- Household Information
- Affidavit of Residency
- Code of Conduct and Computer Network Policy
- Request for Transportation
- New York State Migrant Education Program Parent Survey
- Home Language Questionnaire
- Student Racial and Ethnic Identification
- Photography Policy
- Physical Exam Form
- Dental Health Certificate
- Emergency Health Information
- Request for School Physical
After completing the registration packet and gathering the three residency documents, the next step is to bring this information to the register’s office along with the child’s birth certificate with seal from court (not hospital) or certified copy, health and immunization records. Students who are transferring to VCSD from another district must also provide their most recent report card or transfer notice, and any standardized test scores. After the packet is filled out completely it can be delivered to:

Note: Packets should not be emailed to the district but instead mailed or dropped off to ensure the greatest privacy protection of your student’s information.

Kindergarten Screening
Voorheesville Central School District looks forward to welcoming all members of our next kindergarten community! As we prepare, we want to get to know each child’s unique strengths and needs. The Kindergarten Screening Questionnaire focuses on skills that help children navigate their new classroom environment with confidence.
Please keep in mind that every child develops at their own pace; it is entirely normal and expected to see a wide range of abilities at this age. Whether a child is just starting to learn these skills or has mastered them, honest feedback helps our teachers create a supportive, personalized environment where every student can thrive.
Intent to Home School
Parents and guardians of children who are or will be homeschooled will find important information in the following links about state law, reporting requirements and additional services that are available. It’s important to note that all school age children residing in the Voorheesville Central School District must be registered with the school district before they can file a Notice of Intent to Homeschool.
