Vaccine clinic for students age 12 and up, May 20th at HS Gymnasium

Crestwood Pharmacy and Young's Pharmacy & General Store are partnering up with the Voorheesville School District to hold a COVID-19 Vaccine Clinic prioritizing eligible students (aged 12 years old and above).  

The clinic will be administering Pfizer vaccines on Thursday, May 20th from 1:30 - 4:30 p.m. at the Clayton A. Bouton High School Gymnasium. This pod will be prioritizing students ages 12 and up. Each will be required to complete a copy of a parental consent for which can be found here. To make an appointment, scroll to the bottom of the consent forms. Because this clinic is for students ages 12 and up, we are allowing one parent/guardian to attend with the student. We ask that all students getting the vaccine be well hydrated on the date.

The Pfizer COVID-19 vaccine is a two-part vaccine. Individuals receiving the vaccine on May 20, 2021 will be required to return on June 10, 2021 to receive the 2nd dose. If you are not able to return on June 10, 2021, please do not register to receive the vaccine at this time. There will be more opportunities in the near future.
