As we look ahead to summer activities and then the start of a new school year, parents and guardians are reminded that students must be up-to-date with their physical examinations and immunizations to participate in school and school-based activities or receive working papers.
New York State law requires that each child in a school district have a health examination, which includes body mass index, in the following circumstances:
- Entering the school district for the first time and in prekindergarten, kindergarten and grades 1, 3, 5, 7, 9 and 11;
- Participating in strenuous physical activity, such as interscholastic athletics; and
- To obtain an employment certificate (working papers.)

Share the New York State Department of Health Immunization Requirements for School Entrance/Attendance Chart with your child’s health care provider (MD, NP, PA) so they can provide the immunizations (shots) your child needs. The age at which immunizations are given must match the chart.
